Connecting members with community resources and assistance programs that provide support with common needs such as food, housing, transportation and childcare. Also, provides Physician referrals.
Care Coordination team works together with members to help them with the following:
- Understand health plan benefits and navigate the health care system
- Find a different primary care provider who is the right fit
- Get appointments for physical and behavioral health, dental, home health, durable medical equipment and long-term care needs
- Create personal health and wellness goals
- Learn more about their health conditions and self-management tools to improve their health
- Enroll in programs for chronic disease and overall health and wellness
- Connect with services and programs offering assistance for non-medical needs that impact their health and well-being

|Options and Benefits Counseling - Mesa County Aging and Disability Resources of Colorado||Options counselors provide information and support to adults over 60 and adults over 18 with a disability and their family members as they explore their options for long term supports and services. |Private and Social Health Insurance - Rocky Mountain Health Plans||Rocky Mountain Health Plans provides health insurance plans to individuals and families including those enrolled in Health First Colorado (Colorado Medicaid) and Medicare.
RMHP can also help members if they are having difficulty locating a health provider for both private insurance plans and Medicare or Medicaid. Members can also download provider directories online.
RMHP also provides telehealth/telemedicine services through a platform called CareNow.
