Adult Case Management - Envision | Provides the single entry point for people requesting determination of eligibilty to access developmental disabilities services in Weld County. Determination of an Intellectual or Developmental Disability (I/DD) is required prior to accessing any services through the developmental disabilities system in Colorado. Case Managers work with individuals and families to gather necessary information and to complete intake paperwork for eligibility determination.

Children's Extensive Support Waiver (CES) - Children & Family Services - Mountain Valley Developmental Services | The CES Waiver is a program for Children under the age of 18 with disabilities that require significant medical or behavior support. CES offers a variety of supports that can be selected to address the particular needs of an eligible child and his or her family. These services can include:

Children's Home and Community Based Services Medicaid Waiver (CHCBS) - Children & Family Services - Mountain Valley Developmental Services | The CHCBS Medicaid Waiver is available to Colorado residents who have a child or children with a disability and who do not qualify for Supplemental Security Income (SSI) through the Social Security Administration. Families who qualify for SSI automatically receive Medicaid benefits for their children. SSI eligibility is based on the family’s income and assets.
If a family does not qualify for SSI but needs insurance for their child/ children, this waiver provides Medicaid for the child with the disability even though the parent’s income or assets make them ineligible for SSI, and therefore for Medicaid. | Garfield County | 700 MT SOPRIS DR | Mountain Valley Developmental Services |
Early Childhood Intervention Services - Children & Family Services - Mountain Valley Developmental Services | The Early Intervention Program supports families of children under the age of three, who have been diagnosed with an established condition known to have a high probability of resulting in significant delays in development; or are demonstrating a 25% delay in at least one domain of development by:
- Providing Service Coordination and Case Management Services to connect families to resources
- Coordinating and providing payment for the Therapeutic interventions embedded within each child's daily routines and natural environments

HCBS Waivers for Intellectual and Developmental Disabilities - Colorado Department of Health Care Policy and Financing | Home and Community Based Services (HCBS) Waivers provide assistance for adults and children with intellectual and developmental disabilities.
