- Hospice services for individuals who are diagnosed with a life-limiting illness
- Grief and Bereavement services designed to respond to specific needs of the individuals and families coping with death, loss, and grief.

|Hospice Care - Hospice of the Plains||Hospice provides palliative (comfort) care to individuals whose illness is not curable. Palliative care focuses on making the person as comfortable as possible by managing the symptoms of the disease. |Hospice of the Plateau Valley - HopeWest Hospice & Palliative Care||This Hospice Program is a secondary office in Mes County to serve those up in the Plateau Valley area based at the Plateau Valley Hospital.
Provides comprehensive services to those with life threatening illness, their families and the bereaved. Service could include skilled nursing, all therapies, social work, chaplain, physician services (Hospice or Transitions patient care).

|Hospice - Prospect Home Care & Hospice||Hospice is a program which provides comfort and supportive care for terminally ill patients and their families. The whole family is considered the unit of care, and care extends through their period of mourning. The focus of this care is to enable the patient to remain in the familiar surroundings of their home for as long as possible. To apply for this program you will need a physician referral. |In-Home Services - Mount Evans Hospice and Home Health Care||Provides in-home medical care for individuals who are terminally ill or recovering from illness in Jefferson, Park, Gilpin, and Clear Creek counties. |TRU Hospice Care - Hospice of Northern Colorado||Provides hospice care in patients’ homes, in long-term care facilities, and at the Inpatient Care Center, located on the fourth floor of North Colorado Medical Center.
