    Programs that issue, maintain and, where appropriate, provide access to or copies of the official documents that certify that an individual has died. Death certificates specify the cause of death and are generally signed by an attending or examining physician. Available in most jurisdictions are authorized copies which may be required to obtain death benefits, claim insurance proceeds, notify Social Security and access other services related to an individual’s identity; and informational copies which cannot be used to establish identity. Only individuals designated by law have access to authorized copies of a person's death certificate.

    Birth and Death Certificates - City of DaconoBirth Certificates: Issues certified copies of birth records for births occurring anywhere in Colorado from 1904 to the present. For births before 1904, the county can issue certified copies only on those births that occurred in Weld County. All other certificates must be requested either from the county of birth or from the Colorado Department of Public Health and Environment. Weld County can issue certified copies of death records for all deaths that occurred in Weld County. Fees for certified copies of birth records are $17.75 for the first copy and $10 for each additional copy issued at the same time for the same record.
