    Durango La Plata Senior Center - La Plata Senior ServicesThe Durango Senior Center is dedicated to enhancing the quality of life for all adults age 60 years and older and helping them age with dignity and purpose. Activities and services include: HOME CHORE SERVICES which include: - Installation of grab bars and handrails - Correction of trip hazards - Minor step and porch repair - Small home repairs - Heavy house cleaning - Spring and Fall yard clean-up - Limited snow removal assistance - Firewood for low income families ***Services are limited. We transport seniors from Monterey Park, Ratekin Towers, Nellie Bechtel, Walnut Park, and Grand View locations.     SOS - Supporting Our Seniors Transportation Program - ADRC of Mesa CountySOS is a volunteer based transportation program for seniors age 60+ who are unable to drive and do not have any other means of transportation. Recipients must be able to get themselves in and out of a car with only elbow assistance. Rides are limited to doctor appointments, physical therapy, dental appointments, dialysis, grocery, and pharmacy.     The Traveler - Garfield CountyThe Traveler provides wheelchair accessible, curb to curb, demand responsive, driver-assisted transportation to Garfield County residents, 65 or older or disabled adults (over 18) who have difficulty using public or private transportation services. Based in Glenwood Springs and Rifle, the Traveler serves qualified individuals within the following service area parameters: a five (5) mile radius off of Interstate 70 between Parachute and Glenwood Springs, and a five (5) mile radius off of Hwy 82 between Glenwood Springs and Carbondale.     Transportation - Berthoud Rural Alternative for Transportation (RAFT)Seniors (60 or older) and Adults (18 or older) with disabilities, who reside within the Berthoud Fire Protection District. Rural residents, who do not reside within the town limits will be provided transportation for any legal purpose. By agreement with the Town of Berthoud, town residents who meet RAFT eligibility criteria and need transportation for medical, educational or employment purposes may be transported by RAFT into Longmont or Loveland outside of posted hours of Berthoud Area Transportation Service (BATS). RAFT may not duplicate existing services that are being provided by BATS.     Transportation - Park County Senior Coalition (PCSC)We schedule trips and outings that include shopping and medical appointments, as well as fun and leisure trips. Vans meet at pre-determined locations for pickups. A two-month trip schedule is posted in our bi-monthly newsletter. Volunteers use their personal vehicles to transport clients on medical trips if they are unable to take the van on our regularly scheduled trips. We arrange transportation for individual clients to go to the VA and other hospitals, doctor offices, and more.     Transportation ServicesTransportation is provided to Eagle County residents to and from nutrition sites, medical appointments, and to run errands. Transportation services are available to those over the age of 60 and disabled adults. Senior Services operates van and bus service at all three sites on nutrition days. Minturn: Meal transportation on Wednesday and Friday; shopping on Friday Eagle: Meal transportation, shopping, Post Office and local medical appointments, Tuesday and shopping on Thursday. Basalt: Meal transportation on Tuesday and Thursday; shopping and Post Office on Thursday; medical van to Glenwood Springs from Eagle on the last Monday of each month; medical van also available by appointment. Transportation is provided for Eagle County residents over the age of 60 as well as disabled adults to and from nutrition sites and to run errands. Aging Services operates van and bus service at all three sites, Basalt, Eagle and Minturn. Assistance with transportation to medical appointments can be obtained by contacting Lorene Gallagher at 970-328-1121. Volunteer Transporters assist older adults and disabled persons with rides to and from doctor appointments, shoppping and other tasks when no other forms of transportation are available. The Department of Health & Human Services (HHS) and ECO Transit have joined to issue seniors and disabled adults an identification card which allows the user to ride ECO buses for minimal cost.
