Intake ProcedureCall for specific information or to schedule a ride.
Intake RequirementsIndividuals must provide:
- Medicaid ID number
- Name
- Date of Birth
- Pick up address
- Doctor’s name and address
- Telephone number for confirmation of the appointment
- Date and time of the appointment
- Estimated time for any return trip
Helpful TipsTrip must be within 25 miles of the pick-up location. Must be scheduled at least 48 hours in advance.
