Description of Service
The Senior Care Navigation Program helps senior adults and their families connect with options to insure safety and independence.
Successful care management is accomplished by:
-Identify the needs, concerns and goals of the senior adult
-Assess the personal health and physical environment of the senior
-Develop a care plan by determining needed services and make recommendations based on quality and cost effectiveness
-Locate and coordinate the delivery of services and identify potential resources
-Monitor the provision of services and adjust services as required
Care management is currently provided at no cost to Pitkin County Seniors over age 60.
Urgent Services Provided
Truly urgent situations should be addresses with law enforcement by dialing 911 or Adult Protective Services by dialing 970-429-2040 ext. 1

None required for care management. Requirements may exist for some programs and services eligibility.
English, limited Spanish
Must be age 60+ or the family member of a senior requesting assistance. Must be a resident of Pitkin County.
