Older Adult and Caregiver Services is specifically designed to support older adults and their families as they wrestle with aging-related issues. These issues can range from concern for an aging parent's memory or physical health to finding affordable housing. Mental health counseling for caregivers and seniors dealing with difficult transitions is also available.

Call the intake coordinator for more information; care managers start with a comprehensive assessment of the older adult and their family in the home. A care plan is developed and may involve referrals to other services, help navigating health care systems, transitions to new housing, and assistance with financial and legal issues.
