Description of Service
Health First Colorado (Medicaid) assists with medical expenses for eligible, low-income Colorado resident families and pregnant women. Children's Health Plan Plus (CHP+) is a low-cost health insurance program for eligible children and pregnant women who do not qualify for Health First Colorado (Medicaid).
Complete an application online using PEAK. Access Colorado PEAK at: https://coloradopeak.secure.force.com/
Call or walk in to receive an application by mail or in person.
Documentation that may be required:
- Social Security number for all household members
- Proof of Citizenship and Identification for all household members
If not available, sign and submit an application and the Department will follow-up.
Colorado PEAK provides a questionnaire that will give people answers to help them figure out if they qualify for benefits for programs that provide food assistance, cash assistance, WIC supplemental nutrition benefits, and other types of assistance.
