Description of Service
Housing & Benefits services are available to help the disabled with housing issues/information and Direct Services assistance from housing, nursing home transition, case management, benefit advocacy and application assistance. CFI is not an emergency services organization.
-Assist with disability hearings
-Assist with applications if needed due to the consumer's disability (TANF, Aid to the Needy Disabled, Food Stamps, Medicaid, Medicare, Social Security Benefits (SSI/SSDI), LEAP, Section 8 and other housing applications
-Assist with finding possible housing
-Advocate in an effort to resolve landlord/tenant issues and tenant rights/responsibilities
-Assist with possible housing when being discharged from hospital
-Assist with those wanting to transition out of a nursing home and back to the Community (CCT)
-Work to help prevent possible nursing home placement
-Assist in finding durable medical equipment or adaptive aids and the availability of funding opportunities to help purchase equipment
-Assist with locating vendors who can do home modifications
-Answer questions about the ADA (American Disabilities Act) Fair Housing, Service Animals, Therapy Animals and Companion Animals
-Referrals to appropriate agencies and service providers in the community
