Description of Service
Provides services and case management and prevention services to HIV-positive individuals and those at risk of HIV contraction in 22 counties along the western slope.
Service may include:
-Walk in testing for HIV and Hepatitis C
-Health Care Provider referrals
-Mental Health Counseling
-Food Assistance Referrals
-Syringe Exchange Program
-Peer-driven support groups
Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, Lake, La Plata, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit Counties
