Description of ServiceProvides case management and preventive services to HIV-positive individuals and those at risk of HIV contraction in 22 counties along the western slope of Colorado.

Service may include:
-Walk in testing for HIV and Hepatitis C
-Case management
-Housing referrals
-Health Care Provider referrals
-Mental Health Counseling
-Food Assistance Referrals
-Syringe Exchange Program
-Peer-driven support groups
