Description of Service
Provides free or low-cost health insurance for Coloradans who qualify. Also offers Long-Term Care, which may include nursing facility care and Home and Community-Based Services, which provides care in one's home.

Apply online at Colorado PEAK or walk in to a Human Services office in Aurora or Littleton to apply online at a free computer kiosk, or print an application and fax, mail or deliver it to either the Aurora or Littleton office. Individuals applying for Medicaid, will not need to complete a new application for CHP+ for their family.

PEAKHealth is for Medicaid and CHP+ members who have a Colorado.gov/PEAK account. With PEAKHealth, members may now keep their information up-to-date and access important health information from their phone by downloading the PeakHealth mobile app from the Apple iTunes Store or Android/Google Play App Store for free. Colorado PEAK accounts may be opened and accessed at https://coloradopeak.secure.force.com/.
Individuals and families that meet one of the following categories and income requirements:
- Children ages 0-18 with household income under 260% Federal Poverty Level (FPL)
- Pregnant women, over the age of 19, whose household income is under 260% FPL
- Parents and Caretaker Relatives (you must have a dependent child) whose household income does not exceed 133% FPL
- Adults without dependent children whose household income does not exceed 133% FPL
