Description of Service
Health First Colorado (Colorado’s Medicaid Program) provides eligible low-income individuals and families with medical, medication, nursing home or home and community-based assistance. Interviews or appointments are not required for Health First Colorado.
Applications maybe accepted in person, by fax, by mail, or online. Call for more information.
You may be required to provide proof of:
- Social Security Card
- Proof of income earned/unearned
- Legal residency
- Additional documents may be requested
Individuals may also return applications to the Fort Lupton Campus Office located at 2950 9th St, Fort Lupton, CO 80621.
Medicaid does not pay for a sport’s physical. Please contact your Primary Care Provider and make an appointment to get a Well Child Check and bring the school form with you to have it signed. If your child has had a well child check in the last year, they won’t need one. The provider can choose to sign the forms since one was already done.

Children ages 20 and under who are enrolled in state Medicaid services.
