Description of ServiceHealth First Colorado (Colorado’s Medicaid Program)
Healthy Communities combines aspects of the Early Periodic Screening Diagnostic and Treatment (EPSDT) Outreach and Administrative Case Management program and Child Health Plan Plus (CHP+) outreach into one model to better meet the needs of clients. Family Health Coordinators are available statewide to serve clients through Healthy Communities.
This outreach and case management model takes into account that many of our families do not always understand the distinction between Medicaid and CHP+. In fact, many families have one child that is enrolled in Medicaid while another is enrolled in CHP+.

Healthy Communities focuses on the "life cycle of a client” which may involve all of the activities necessary for the client to obtain coverage and access to coordinated health care services in appropriate settings called a Medical Home. Family Health Coordinators perform the following activities:
Urgent Services ProvidedGenerate awareness of the existence of the Medicaid and CHP+ programs;

Offer information about how to apply for Medicaid and CHP+ and the availability of face-to-face application assistance;
Inform families about where to submit applications for processing and eligibility determination;
Educate families on the value of preventive health care services and how to access benefits at the appropriate settings;
Link clients to Medicaid and CHP+ providers who will serve as the client's Medical Home;
Provide clients with information and referrals to other community programs and resources; and
Explain the re-enrollment process to families who continue to be eligible for Medicaid and CHP+ to eliminate gaps in coverage.

Helpful TipsMedicaid does not pay for a sport’s physical. Please contact your Primary Care Provider and make an appointment to get a Well Child Check and bring the school form with you to have it signed. If your child has had a well child check in the last year, they won’t need one. The provider can choose to sign the forms since one was already done.

EligibilityHealthy Communities:
All Colorado citizens who would like to access public programs or those who are already eligible for public programs and are under the age of 21 or pregnant.

Dental Services:
Children ages 20 and under who are enrolled in state Medicaid services.

Medicaid does not pay for a sport’s physical. Please contact your Primary Care Provider and make an appointment to get a Well Child Check and bring the school form with you to have it signed. If your child has had a well child check in the last year, they won’t need one. The provider can choose to sign the forms since one was already done.
Must be a resident of the County they are applying in, and meet income and resource eligibility guidelines of the State and Federal government.
