Health First Colorado (Colorado’s Medicaid Program)
Provides Family Health Coordinators available statewide to serve clients through Healthy Communities. Offers outreach and case management model that takes into account that many families do not always understand the distinction between Medicaid and CHP+. In fact, many families have one child that is enrolled in Medicaid while another is enrolled in CHP+. Focuses on the "life cycle of a client” which may involve all of the activities necessary for the client to obtain coverage and access to coordinated health care services in appropriate settings called a Medical Home.
Applications maybe accepted in person, by FAX, by mail, or online. Call for more information.
You may be required to provide proof of:
- Valid picture ID
- Social Security Card
- Proof of income earned/unearned
- Legal residency
Medicaid does not pay for a sport’s physical. Please contact your Primary Care Provider and make an appointment to get a Well Child Check and bring the school form with you to have it signed. If your child has had a well child check in the last year, they won’t need one. The provider can choose to sign the forms since one was already done.
All Colorado citizens who would like to access public programs or those who are already eligible for public programs and are under the age of 21 or pregnant.
Children ages 20 and under who are enrolled in state Medicaid services.
Must be a resident of the County they are applying in, and meet income and resource eligibility guidelines of the State and Federal government.
