Children's Case Management and Early Intervention - Envision

Description of Service
Provides the single entry point for people requesting eligibility for early intervention and developmental disabilities services in Weld County. Determination of a Developmental Delay (DD) up to age 5, or Intellectual or Developmental Disability (I/DD) from age 5 and on is required prior to accessing any services through the developmental disabilities system in Colorado. Case Managers work with individuals and families to gather necessary information and to complete intake paperwork for DD or I/DD determination.
For individuals who are also applying for Medicaid Waiver services, including the Children with Autism waiver (CWA) or the Children’s Extensive Support waiver (CES), Case Managers complete the required assessment with the child(ren) and caregivers. Case Managers are responsible for assuring that all individuals are afforded due process.
