General Information
Description of ServiceCombines aspects of the Early Periodic Screening Diagnostic and Treatment (EPSDT) Outreach and Administrative Case Management program and Child Health Plan Plus (CHP+) outreach into one model to better meet the needs of clients. Family Health Coordinators are available statewide to serve clients through Healthy Communities.
This outreach and case management model takes into account that many of our families do not always understand the distinction between Medicaid and CHP+. In fact, many families have one child that is enrolled in Medicaid while another is enrolled in CHP+. Focuses on the "life cycle of a client” which may involve all of the activities necessary for the client to obtain coverage and access to coordinated health care services in appropriate settings called a Medical Home. Family Health Coordinators perform the following activities:

- Valid picture ID
- Social Security Card
- Proof of income earned/unearned
- Legal residency
- Citizenship
Helpful TipsMedicaid does not pay for a sport’s physical. Please contact your Primary Care Provider and make an appointment to get a Well Child Check and bring the school form with you to have it signed. If your child has had a well child check in the last year, they won’t need one. The provider can choose to sign the forms since one was already done.
