General Information
Description of ServiceAging in Place provides Senior Service Coordinators who advocate and coordinate services for seniors residing in the Denver Metro area. Primary aim is to connect vulnerable seniors to benefits and valuable resources that allow them to live independently for as long and safely possible. Senior Service Coordinators will screen for benefits and connect seniors with services such as:

- Application assistance for Medicaid/Medicare
- Affordable housing search
- Durable medical equipment loan closets
- Financial assistance for hearing aids, dentures and eyeglasses
- Food banks and food delivery services
- Free companion care/housekeeping
- Home and Community-Based Services
- Low-cost telephone and internet services
- Low-vision resources

Intake ProcedureIntake applications can be completed over the telephone, in-person at the office or in the senior's home.
Intake RequirementsIndividuals who reside in the service areas must provide the following documentation:
- Photo ID
- Contact information
- Demographic Information
- Service Needs
- Activities of Daily Living (ADLs)

EligibilitySeniors ages 60 and older residing in the service areas. However, individuals out of the service areas are eligible for assistance.
