Description of ServiceProvides health care coordination services for adults with intellectual and developmental disabilities. Service include nurses monitoring medical files, monitoring and managing acute and chronic medical conditions, providing in-home care, coordinating benefits, coordinating appointments with doctors and specialists, and providing transportation and accompanying individuals to medical appointments.
