Description of Service
Provides comprehensive services for people with disabilities in order to foster and maintain independence by bringing the services – personal and medical – to their homes. By doing so, it reduces the risk or need for people with disabilities to unnecessarily transition to nursing homes or be over-dependent on family members.
CPWD's Home Health Services also include In Home Support Services (IHSS). This is a consumer-directed program in which CPWD oversees caregivers, but the consumer hires caregivers of choice and creates his or her own care plan. The caregivers are not required to be licensed, but can do care, as directed by the consumer, which may include tasks normally assigned to a CNA or even an RN.
Home Health Services Include:
- Skilled Nursing (medication management, injections, health and safety monitoring, etc.)
- Certified Nursing Assistants (CNA) (bathing, toileting, feeding, etc.)
- Personal Care Provider (PCP) (medication reminders, oral care, exercise, etc.)
- Homemaker Assistance (light cleaning, cooking, laundry, etc.)

In addition to providing Home Health Services, CPWD can assist family members in becoming Caregiver Certified Nursing Assistants (Caregiver CNAs). This is an opportunity for family members to get paid to care for their loved ones. For more information, please contact the Home Health Administrator.
