Description of Service
The Center for People with Disabilities offers a variety of programs to assist consumers in achieving independence, including the following services:
- Employment program that offers comprehensive job training and support
- Youth or Adult Transition Program that assists individuals in transitioning into home living from nursing care or medical institutions
- Day program with skills training, peer support, and care coordination
- Core Services
- Beyond Vision Program, which provides support services, community outreach, and facilitates support groups for individuals who are blind or visually impaired
Call for more information or to discuss if a program or service will be a good fit. If so, an appointment will be made with an Independent Living Adviser who will start the intake process; during the intake, goals will be established and client information will be gathered.
Please arrive on time for the intake appointment.
Because CPWD is a consumer controlled organization (51% of the Board and staff are people with disabilities), requests for services MUST come from the individual or parent, NOT a referral source.

For the Day programs clients will need to have an HCBS waiver.
