Intake ProcedureCall to schedule an appointment.
Intake RequirementsWill need to bring valid ID for the adults in the home, last 4 digits of the Social Security numbers for everyone in the home, proof of income, and a disconnect notice from the utility company.
Additional Requirements**Nov 1- April 30 you MUST apply for LEAP and show proof of approval for LEAP before you can receive COPE assistance.**
