Description of ServiceProvides temporary housing for homeless persons to medically recover when released from local hospitals or medical rehab facilities.

Intake ProcedureA referral is needed from a local hospital or rehab facility. Referrals should be done when the patient is near or ready for discharge. Patients need to be independent of ADL's, not detoxing, and not a sex offender. Go online to download a referral. Fill out the referral and fax back to the program.
