Description of ServiceProvides prescription assistance once in a 12-month period, as funding is available.

Intake ProcedureCall to make an appointment. At the appointment, sign in and present the required documents to the Admissions Coordinator. Client must meet with a case manager. The prescription must be filled only after assistance has been approved at a designated King Soopers’ pharmacy. IFCS cannot pay for prescriptions filled at any other location.
Intake RequirementsPhoto ID for every adult 18 and older in a household for first intake; Head of household's ID for each subsequent visit.

EligibilityClients must live in the service area and their household income must fall within 185% of the Federal Poverty Level.
