Description of ServiceProvides eligible clients with a voucher for an eye exam and free pair of eye glasses at participating optical offices once in a 12-month period when vouchers are available.

Intake ProcedureCall to make an appointment. At the appointment, sign in and present the required documents to the Admissions Coordinator. The need must be verified by the One Vision Coordinator.
Intake RequirementsPhoto ID for every adult 18 and older in a household for first intake; Head of household's ID for each subsequent visit.

EligibilityClients must live in the service area and their household income must fall within 185% of the Federal Poverty Level.
