Assistance through ADRC is meant to streamline access to long-term care information and services for people age 60 and over and age 18 and over with a disability that have long-term care needs.
Options counseling is a person-centered approach to help individuals gain an understanding of the benefits and limitations of long-term care support and service options.
Options counselors can assist clients with:
- Interviewing and assessment of needs
- Exploring individualized options and resources
- Decision support
- Assistance in accessing services
- Action planning
- Follow up to assure service delivery and quality of care
