Description of Service
Colorado Medicaid Non-Emergent Medical Transportation (NEMT) will assist Pueblo County Medicaid recipients with non-emergency medical transportation to and from their scheduled Medicaid doctor appointments for those who have no other means of transportation including free transportation. This service is a benefit provided by Colorado Medicaid. Mode of transportation must be least costly.
MODES OF TRANSPORTATION
-Mileage Reimbursement for your own private vehicle use to and from your doctor appointments.
-City Bus Round Trip Tickets or a 22-ride Pass for the month.
-Citi-Lift monthly pass. You must also be approved through Pueblo Transit Citi-Lift program and provide your registration number to NEMT to receive a pass.
-Taxi-Cab or Privately Contracted Vehicle services through:

-Non-emergency ambulance as medically required for transport. Which must be indicated on medical certification application
Services NOT covered:
Picking up prescriptions, medical
equipment, non-medical rides,
emergencies, or completing interviews or
Pueblo County, R6
Call and request a NEMT application packet and leave your name, Medicaid number, and mailing address to request the application.
Medicaid recipients with no other means of transportation.
Medicaid programs that may be eligible for NEMT include:

-MAGI Adult, Children, Pregnant
-Old Age Pension Med-A
-Child Welfare Medicaid
