Description of Service
Provides a free, evidence-based community program designed to help improve the health, well-being and self-sufficiency of first-time mothers and their children. Participants in the program receive home visits from a registered nurse until their baby is two-years old.

Eligibility
First-time mothers who are 28-weeks pregnant or less and who meet income eligibility requirements.
