Description of Service
Provides public health insurance for Colorado residents who qualify based on family size and income. Individuals, families, children, pregnant women, and persons who are blind, disabled or elderly may qualify. Individuals who receive Medicare may also qualify for Medicaid to assist with Medicare premiums.

- Social Security numbers for all household members
- Proof of income for non-citizens and self-employed persons
