Intake ProcedureCall to schedule an appointment. No walk-ins accepted.
Intake RequirementsNew clients must present:
- IDs for every member in the household (e.g., Driver's License, Birth Certificate, Medicaid Card, Social Security Card, Passport, or Prescription Bottle).
- Piece of mail within the last 30 days with the name of head of household and address.
