Client Information
EligibilityHome Health Services: No Restrictions Applied; Medicaid Waiver participants through the SLS or COMP programs must have prior authorization through their Case Manager at their applicable CCB.

Independent Living Home: Adults with developmental/intellectual disabilities with Medicaid Waivers through the SLS or COMP programs. Must have prior authorization through their Case Manager at their applicable CCB.
Hours of OperationMonday - Friday, 7 a.m. - 6 p.m.; Saturday, Sunday, 10 a.m. - 4 p.m. (Office Hours); 24 hours/7 days a week (On Call)
