Intake ProcedureWhen a consumer is referred, a case manager is assigned and an appointment is set up to gather information.
Intake RequirementsMay be asked to provide the following:
- School records
- Psychological tests
- IQ score
- Applicable scores
- Birth certificate
- Medicaid card
- Social Security card
