General Information
Description of ProgramThis service is designed to assist adults with severe and persistent mental illness, assisting them to achieve greater independence in the least restrictive environment and avoid psychiatric hospitalizations. Services include intensive case management, vocational services, entitlement acquisition, individual therapy, support groups, skills building, payee services, and medication management and monitoring.

Client Information
Intended ParticipantsBecause primary funding is through state and federal monies, via a medicaid contract, we provide services primarily to those on Medicaid. However, we also offer sliding scale fees, based on ability to pay. Inidviduals over 18 with a severe and persistent mental illness are our target audience.
