General Information
Description of ProgramProvides support and early intervention services to families with children ages birth to three who are developmentally delayed or disabled, as well as services for adults ages 21 and older with developmental disabilities and a funding source.

The central Horizons office is located in Steamboat Springs, with programs serving Routt, Moffat, Rio Blanco, Jackson, and Grand counties.

Client Information
EligibilityChildren ages birth to three, adults ages 21 and older with intellectual/developmental disabilities (IDD), and students or adults of any age with IDD living at home.

Hours of OperationMonday - Friday, 8 a.m. - 12 p.m., 1 p.m. - 5 p.m.
