Description of AgencyOffers free Hepatitis C and HIV testing and Linkage to Care through the Patient Navigator program. Liver Health Connection also provides community education, patient support, liver disease prevention strategies, and educational and support opportunities for people living with hepatitis C, their families and friends, and healthcare providers.
MissionTo promote liver health, provide education, and to support and advocate on behalf of those affected by liver diseases.
HistoryEstablished in 1995, Liver Health Connection (formerly Hep C Connection) is one of the nation's only hepatitis C-focused advocacy organizations and the only one of its kind in Colorado.
