Description of AgencyProvides a free, evidence-based community program designed to help improve the health, well-being and self-sufficiency of first-time mothers and their children. Participants in the program receive home visits from a registered nurse until their baby is two-years old.
MissionEmpower first-time mothers living in poverty to successfully change their lives and the lives of their children through evidence-based nurse home visiting.
