General Information
Description of AgencyHospice is both a philosophy and program providing compassionate and quality care for persons with limited life expectancy and those caring for them. Services include: In-home hospice and palliative care (24-hour access), In-patient hospice care at the skilled nursing and long-term care facilities,Joni Fair Hospice House, a 10 bed in-patient care unit, Home Health Aide Care, social worker services, chaplain services, volunteer services, bereavement services, support programs for care-givers.
MissionOur mission is to provide terminally ill patients with the necessary physical, emotional and spiritual support to ensure the best quality of their remaining lives, allowing them to die with dignity and to provide their loved ones with emotional support.

Intake ProcedureReferrals from anyone, we will contact patient's primary physician for certification of terminal illness and order to admit to hospice, Then we contact patient's family, and admission team makes visit to place of residence and will sign if appropriate.
Intake RequirementsA life threatening illness, and referral by a physician for hospice program, Prognoses of 6 months or less, patient's family must desire hospice support & care.
Funded ByMedicare/Medicaid, private insurance, sliding fee scales, donations and grants.
